What is a Medical Report?

A medical report is a document that you may encounter in many official applications. In addition to being a report that proves you are healthy, a medical report can also be a report that identifies your illness or ailments. From a legal point of view, a medical report is a document created and approved by an authorised physician in accordance with certain criteria, regarding whether the person’s physical and mental health is suitable for performing a certain job, task or purpose.

Why You May Need a Medical Report?

You may need a medical report at many stages of your life. You can request a medical report from our authorised physicians at Private Özkaya Medical Center when getting married, getting a driver’s license, getting a new job, applying for a boarding school or dormitory, or in other cases mentioned below.

A medical report for a driver's license, also known as a driver's license report, is a required document when applying for a driver's license for the first time, renewing your driver's license or adding a category, and in cases where you lose your driver's license.

As of 2016, the license categories and the conditions for obtaining reports have changed. For this reason, the concepts of 1st group medical reports and 2nd group medical reports have emerged.

The 1st group medical reports cover M, A1, A2, A, B1, B, BE and F category licenses. These categories consist of lightweight vehicle drivers, and drivers in this category are required to renew their driver's licenses with a new medical report every 10 years.

2nd group medical reports cover C1, C1E, C, CE, D1, D1E, D, DE and G class driving licenses. These categories usually consist of heavyweight vehicle and commercial vehicle drivers, and drivers in this category are required to renew their driver's licenses every 5 years by obtaining a new medical report.

You can get the medical report for your driver's license by applying to the Private Özkaya Medical Center, as well as to public hospitals, your family doctor or other private health institutions.

An incapacity for work report is a report that an insured employee must receive with the approval of a specialist doctor if they become ill, become pregnant, have a work accident, or suffer from an occupational disease. In order to receive incapacity benefits by the SGK, it is mandatory for patients to receive an incapacity for work report.

According to the Social Insurance and General Health Insurance Law No. 5510, there are 3 different temporary incapacity benefits. These can be classified as having an accident at work or contracting an occupational disease, illness or discomfort, and pregnancy and childbirth for women.

There are certain documents that an insured female employee in a workplace should prepare for during pregnancy and before delivery. While the Incapacity Report is one of them, another important document is the Prenatal No Objection To Work Report.

If the female employee wants to work up to 3 weeks (approximately 37 weeks) before the estimated due date, she can notify her doctor about this request. If her doctor finds it appropriate after the examination, she can get a "Prenatal No Objection To Work Report" in her 32nd week.

The point to be noted here is that when the female employee completes her 37th week of pregnancy, she must obtain an "Incapacity Report". The Prenatal No Objection To Work Report is given by our specialist physicians after the examination in our Gynecology and Obstetrics unit.

Individuals with chronic diseases may need to use medication on a regular basis. In the treatment of diseases that require constant use of medication such as blood pressure, diabetes, cholesterol, doctors can give continous medication reports.

After the examination to be performed by our doctors at Özkaya Medical Center, you can receive your continuous medication report if deemed appropriate. This report can be for 3 months, 6 months or 12 months old.

When the report is given to you, you need to print out a prescription in order to receive the medicine. There is no need to carry this report with you with a prescription print out and you can pick up your medication by stopping by any pharmacy with your prescription.

Students who want to enroll in boarding schools or student dormitories are required to obtain a health report after a doctor's examination.

The reason for requesting a health report when registering at the dormitory or school is to determine that it will not infect other students who will reside in the dormitory or school. For this reason, it is obligatory for the person who wants to get a health report to personally apply to the health center.

Before applying to the health center, two passport-sized photographs taken within the last 6 months and a photocopy of the front and back of the identity card are required.cards are required. Another name for the health board report is the committee report. A health board report can be composed as a result of the examination of at least three different branch experts. A legal capacity report, also known as a mental faculties report, is a health report that can be requested from individuals over the age of 65 for official procedures. The reason why this report exists and is used is to prevent people who want to take advantage and benefit from the difficult, powerless situation that elderly people are in.

The legal capacity report can be requested during the title deed transactions, marriage proceedings or power of attorney procedures. For the report, a person over the age of 65 should first be evaluated by a psychiatrist, and then undergo some tests by a neurology specialist, if deemed necessary.
